CFPD Private Services is the fee-for-service component of the Colorado Fund for People with Disabilities.  We draw on the years of experience and expertise of CFPD, which is a pooled, supplemental needs trust.

Who We Serve

The staff at CFPD works with people with a wide range of disabilities, from developmental disabilities to physical disabilities or mental illness.  We can coordinate the many services involved in the individual's daily life, such as residential, medical, financial and vocational.  Although our knowledge and experience base is wide, we are always focused on serving the needs of each individual.

Private Case Management

       ~  Long-term/Ongoing

CFPD is committed to establishing long term relationships with individuals to secure quality care, services and support throughout their lifetime.  CFPD utilizes its years of experience and its strong connections in the community to assist clients in developing a comprehensive plan for their care.  CFPD helps to implement and facilitate these plans by linking people with quality providers and resources throughout Colorado and coordinates these services for individuals.

We are also well versed in Supplemental (Special) Needs Trusts.  We will work collaboratively with the beneficiary, their family members, trustee and trust department,  as well as service providers, attorneys and other supports to use the Supplemental Needs Trust to meet the needs of the beneficiary while maintaining their eligibility for Public Benefits and improving their quality of life.

                Benefits Analysis

Through evaluating your loved one with a disability, we can assist you in navigating public benefits and exploring the person's eligibility for other services.

                As part of the Court Process

Our Assessment and Planning can help the court decide how to best meet the needs of your child or loved one with a disability (Eg: spouse or child in a divorce settlement) through evaluating the individual and recommending funding needed and services available.

                Planning for Future Needs

CFPD recognizes that financial and estate planning for the needs of children is extremely important.  Parents of children or adult children with disabilities face even more challenges.  The special needs of the child must be planned for with extra care while keeping in mind their eligibility and potential need for public benefits and services in the future.  We can help connect you to other professionals in the area of financial and estate planning who are committed to the needs of people with disabilities. 

       ~ Respite/Referral

The CFPD Case Manager can help you find service providers you or your family member will need for ongoing support in your home or in the community or for short-term respite care.  You benefit from CFPD's many years of experience in networking with service providers, especially related to using a combination of benefits and trusts or private funds to provide for these services.

       ~ Funds Management

CFPD has many years experience in trust fund management.  We can assist a trustee in assessing the needs of the beneficiary and making disbursements from their trust or other accounts.

Conservatorships

CFPD excels as Conservator in cases involving people with disabilities, the elderly or other individuals who need a long-term relationship.  Our expertise in fund investment and management, coupled with our Assessment and Planning for individuals gives us a unique advantage in serving as Conservator. 


Trust Advisory Services

CFPD works with trustees and bank trust department to make recommendations related to Supplemental (Special) Needs Trusts.  In this case, the trustee/bank can focus on investments and we use our expertise to help maintain the beneficiary's public benefits such as SSI and Medicaid.  We can also assist a family member who wants to act as trustee by advising them on an as needed basis, re: the impact of trust disbursements on SSI and Medicaid benefits.

Representative Payee

CFPD will manage the Social Security check or other income as Representative Payee for CFPD Beneficiaries and Private Services clients.
